seen so far uses the right chords, so here is my contribution.......

the 'Em/d' is actually just an Em7

Em/d is an Em chord played with a D in the bass note. An Em7 has all the same nots but the e is the bass note and the d (flat 7th of the E scale) is played above the root. Any questions?
