Had breakfast with Ray Sawyer and others from "The Medicine Show" at a little truck stop outside Nashville some 30 years ago. The verse as I remember it being explained to me was:

"I got a freaky old lady name of Cocaine Katy
Who embroiders(pronounce it however you want)on my jeans"
